Through the Life Choices System Transformation efforts and in accordance with Federal Home-and Community-Based Services Medicaid requirements, the Division of Developmental Disabilities is implementing a new person-centered planning process for community-based Medicaid Waiver services effective July 1, 2017. To aid in this implementation, we have created this web page to share important documents and information with consumers, providers, independent service coordinators, and other interested parties.
