The Resource Directory and Guide is a tool for use by transition coordinators assisting members of the class of citizens identified by the Williams Consent Decree. It is the role of the transition coordinator to help the class member secure key resources as he or she prepares to exit an IMD facility and begin independent living in a community of their choice. The Decree calls for preparation in several life areas, such as housing, furnishings, entitlements, health care, mental health care and other domains.
