Medicaid Waiver Services Adults with I/DD – DHS – Division of DD

Eligibility:

Persons 18 and older with IQ of 70 or below prior to age 18 OR related condition before age 22 with significant life skill deficits in 3 of 6 major life activities (self-care, language, learning, mobility, self-directions, capacity for independent living) and PUNS selected. https://hfs.illinois.gov/medicalclients/hcbs/dd.html 

Services:

  • Community Integrated Living Arrangement (CILA), group/host home (24/7 care) or intermittent/in home-family CILA (<24/7 care)
  • Community Living Facility with 16 or fewer residents

Register on waiting list (PUNS) through your Independent Service Coordination agency. Find your ISC here: https://www.dhs.state.il.us/page.aspx?module=12 and choose Developmental Disability Services as Office Type in drop-down menu. 

Intermediate Care Facility for Adults with Developmental Disabilities IDPH

Eligibility:

Persons 18 and older with an intellectual and/or developmental disability or related condition, who have a comprehensive evaluation by an interdisciplinary team and need continuous habilitation services. Not a Medicaid waiver. 

Services:

ICF/DD – which is a type of nursing home with specialized and generic training, treatment, health & related services directed toward the acquisition of the behaviors necessary for functioning with as much self-determination and independence as possible and the prevention or deceleration of regression or loss of current optimal functional status.

Supportive Living Program Persons with Medicaid Managed Care – HFS

Eligibility:

Persons age 22- 64 with a physical disability per Social Security Admin. or persons age 65 or over (provider must designate service to one or the other population); in need of nursing facility level of care & Supportive Living Program meets needs. Cannot be on other HCBS waiver. See https://hfs.illinois.gov/medicalclients/hcbs/slf.html  

Services:

Apartment-style housing with intermittent nursing services, meals/snacks, medication oversight, personal care, social/recreational programming, health promotion & exercise programming, 24-hour response/security staff, emergency call system, laundry, housekeeping, maintenance, well-being check, ancillary services.

Medicaid Waiver Services Persons who are Elderly IL Dept. on Aging

Eligibility:

Persons 60 and older who are at risk of nursing facility placement as measured by Determination of Need (DON) assessment; U.S. citizen or legal alien, Medicaid eligible. https://hfs.illinois.gov/medicalclients/hcbs/elderly.html 

Services:

In home Service (homemaker), Adult Day Service, Emergency Response Service
